Billing Information:
Please bring a valid photo ID and current insurance card to every appointment.
Insurance Claims:
- We file insurance claims for our patients as a courtesy.
- To avoid any delays in processing your claim, it is imperative that we have accurate and current information on file for both the patient and the insured.
- If you do not have the required information available at the time of your appointment, we ask that you pay for the services in full.
- You are then responsible for filing the insurance claim afterwards.
Co-pays & Deductibles:
- All co-pays and deductibles are due at the time of service, as required by your insurance carrier.
Final Financial Responsibility:
- Please review the “Explanations of Benefit” summary you receive from your insurance company after a claim has been processed.
- Any leftover balance, after the insurance company has submitted payment to the service-provider, is the amount you owe.
- The final financial responsibility for payment of services lies with the patient, not the insurance company.
- Please contact our office with any questions you have regarding your bill.
